Position:  Dental Department – Bilingual Patient Registrar

SUMMARY:  The Patient Registrar works performing those duties necessary to prepare patients administratively for a dental visit. Must be bilingual (Spanish-English).  Prior experience in a dental office.

Position:  Behavioral Health Care Manager

SUMMARY: The behavioral health care manager is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the whole patient’s care, substance abuse, mental and physical health care of patients on an assigned patient caseload (based on acuity) with the patient’s medical provider and, when appropriate, other behavioral health providers. This position will also focus on assuring individuals identified with a substance abuse and/or mental health disorders receive treatment, prevention and awareness of opioid abuse.

Position: Certified Medical Assistant

SUMMARY:  The Certified Medical Assistants’ primary duties include assisting the Center’s physicians, mid-level providers, and nurses in the provision of patient care as well as providing non-clinical support for the clinic. The CMA facilitates the flow of patient information across the continuum of care. The CMA prioritizes multiple requests from patients, staff and physicians. The CMA performs a variety of other department specific tasks to ensure optimal department safety and functioning.

Position: Licensed Clinical Social Worker

SUMMARY:  The Licensed Clinical Social Worker, is a Masters Level Licensed Social Worker who is responsible for Assessment and Planning, Direct Services Delivery, Indirect Service Delivery, Evaluation and Education. The employee is expected to provide services that reflect the ethics and values as promoted by the National Association of Social Workers.
